2006 CHS Football Pre-registration Form!

(same form as last year)

The optional Pre-registration form provides Ensign Images with the information needed to shoot your athlete.  This fee is not a deposit, but is a way to prioritize which individuals and events to shoot.  With nearly 100 participants, it is essential that I know who is interested in prints at each level of participation.

Football - the overall number of pre-registration forms determines how many JV and Freshman games we do in addition to most regular season home games.
